Total and Free Testosterone

Testosterone circulates in two forms: bound (attached to proteins, inactive) and free (unbound, biologically active). Most labs only measure total testosterone — but free testosterone is the functional form. Both values together give the most complete picture of hormonal status.

Total Testosterone
Measures all testosterone in the blood — both protein-bound and free. Normal adult male range: 300–1,000 ng/dL. Normal female range: 15–70 ng/dL. Low total T is the primary diagnostic criterion for hypogonadism in men.

Free Testosterone
The biologically active fraction (roughly 1–3% of total T). Some men have normal total T but low free T because high SHBG (sex-hormone binding globulin) binds excess hormone, causing symptomatic testosterone deficiency despite "normal" total T.
