Yellow Fever Vaccine & Certificate (Nigeria Guide)

Yellow fever vaccination requirements, where to get vaccinated in Nigeria, and certificate validity

Yellow fever is a viral haemorrhagic fever transmitted by Aedes mosquitoes โ€” the same mosquito that transmits dengue and Zika. Nigeria experiences periodic yellow fever outbreaks โ€” between 2017โ€“2019 there were significant outbreaks in several states. The yellow fever vaccine is one of the most effective vaccines ever developed โ€” a single dose provides lifetime protection. The International Certificate of Vaccination or Prophylaxis (ICVP, or 'yellow card') is required for entry to many countries and is mandatory for international travel from Nigeria.

Signs and Symptoms

โœ“ Mild yellow fever: fever, headache, muscle pain, nausea, vomiting โ€” resolves in 3โ€“4 days
โœ“ Severe yellow fever (15% of cases): high fever, jaundice (yellowing of skin and eyes), bleeding from gums and nose, vomiting blood, liver and kidney failure
โœ“ Silent phase: symptoms briefly improve, then return severely in the 'toxic phase'
โœ“ Case fatality rate: 20โ€“50% in severe disease

Risk Factors

When to Seek Help

EmergencyFever with jaundice and bleeding in an unvaccinated person from an endemic area โ€” suspect yellow fever, isolate from mosquitoes
Pre-travelBook yellow fever vaccination at least 10 days before international travel โ€” certificate becomes valid 10 days after vaccination
For all NigeriansIf not vaccinated โ€” get vaccinated at your nearest approved centre. Yellow fever is endemic in Nigeria โ€” protection is for your own health, not just international travel requirements

Tests & Diagnosis

๐Ÿงช RT-PCR for yellow fever virus
Confirms active yellow fever infection โ€” available at NCDC reference laboratories
๐Ÿงช Yellow fever IgM antibody
Detects recent infection or confirms vaccine response
๐Ÿงช Liver function tests
Elevated bilirubin and transaminases in yellow fever hepatitis
๐Ÿงช Full blood count
Leucopenia (low white cell count) is characteristic of early yellow fever

Treatment Options

1
Yellow fever vaccine โ€” 17D live attenuated vaccine
A single dose of yellow fever vaccine provides lifetime protection (formerly 10 years โ€” updated by WHO in 2016). Safe for most adults and children โ‰ฅ9 months. Do NOT give to: infants under 6 months, immunocompromised individuals, pregnant women (unless absolutely necessary and risk outweighs benefit), severe egg allergy
2
Where to get vaccinated in Nigeria
NAFDAC-designated vaccination centres โ€” these are the ONLY facilities authorised to issue internationally valid ICVP certificates. Major centres: LUTH (Lagos), UCH (Ibadan), UATH (Abuja), AKTH (Kano), OATH (Ile-Ife), Port Harcourt Teaching Hospital, Murtala Mohamed Airport clinics. Some state general hospitals. Full list on NAFDAC website
3
Cost in Nigeria
Yellow fever vaccination is FREE at some federal government facilities. Private or airport clinics charge a fee. The ICVP certificate itself may have an administrative charge
4
Vaccine validity and certificate format
The International Certificate of Vaccination (yellow card) is issued on an official WHO-standard document. It must be stamped, signed by an approved vaccinator, and show the batch number. A photograph or photocopy is not valid โ€” the original must be presented. The certificate is valid for life from 10 days after vaccination
5
Countries requiring yellow fever certificate from Nigerians
Most West African countries require it from Nigerian travellers. Saudi Arabia for Hajj/Umrah. Many Asian countries. The USA and UK do not require it for entry but airlines may ask. Check your specific destination's requirements with the embassy or IATA Travel Centre
6
Yellow fever in pregnancy
The vaccine contains a live attenuated virus โ€” generally avoided in pregnancy unless the risk of yellow fever exposure is very high (travelling to an active outbreak zone). Discuss with your obstetrician. The disease itself is far more dangerous than the vaccine for the pregnant woman
7
No specific antiviral treatment for yellow fever
Treatment is supportive โ€” rest, fluids, paracetamol for fever (not aspirin โ€” bleeding risk). ICU care for severe disease. Ribavirin has not shown benefit. Prevention through vaccination is the only effective strategy

Frequently Asked Questions

How long is the yellow fever certificate valid?
Since 2016, a single yellow fever vaccination is valid for life โ€” the WHO extended the validity from 10 years to lifetime. If you have an old 10-year certificate that has expired, you need a new vaccination and new certificate. Certificates issued after 2016 for a first-time vaccination are marked as lifetime validity.
Can I get my yellow fever certificate online?
No โ€” a legitimate yellow fever certificate (ICVP) cannot be issued online without in-person vaccination at a designated centre. Certificates purchased online without vaccination are fraudulent โ€” they will not pass entry checks at international airports, which now use electronic verification systems, and they leave you unprotected against yellow fever. Only get vaccinated at a NAFDAC-approved centre.
Is yellow fever the same as malaria?
No โ€” they are completely different diseases, both transmitted by mosquitoes but by different species. Malaria is caused by a parasite (Plasmodium) transmitted by the Anopheles mosquito โ€” treated with artemisinin-based drugs. Yellow fever is caused by a virus transmitted by the Aedes aegypti mosquito โ€” prevented by vaccine, no specific antiviral treatment. Both are serious, and both require mosquito protection.
I was vaccinated years ago โ€” do I need a new yellow fever shot?
If you received a yellow fever vaccine after 2016 and have a valid certificate, you do NOT need revaccination โ€” it is lifetime protection. If you were vaccinated before 2016 with a certificate that shows a 10-year expiry, you will need revaccination if the certificate has expired AND you are travelling to countries that check. Get a new vaccination and new lifetime certificate.
