Difficulty conceiving is common and, in a large share of cases, explainable. The single most useful thing is to stop guessing and establish the basics: whether ovulation is happening, whether the tubes and uterus look normal, and whether the sperm count is adequate. Roughly a third of causes are male, so testing only the woman wastes months.

What usually causes this

Ovulation problems

Including PCOS. Irregular or absent periods are the main clue.

Tubal or uterine factors

Fibroids, adhesions or blocked tubes. A pelvic scan is the starting point.

Male factor

Around a third of cases. A semen analysis is quick and should be done early, not last.

Age-related egg reserve

AMH testing gives an indication of remaining ovarian reserve.

Unexplained

In a proportion of couples no cause is found, which is itself useful information.
