Can't walk up stairs or hurry without getting breathless — may indicate anaemia, heart failure, asthma, or pulmonary disease.

Causes

Common Causes

Anaemia — most common cause in Lagos women

Severe anaemia (haemoglobin below 7–8 g/dL) causes breathlessness because the blood cannot carry enough oxygen to meet demand. Common causes in Lagos: iron deficiency from heavy menstrual bleeding, malaria, and poor dietary iron. An FBC is the first investigation.

Asthma and COPD

Obstructive lung disease causes breathlessness with wheeze — typically episodic in asthma, progressive in COPD. Peak flow measurement and response to salbutamol bronchodilator help confirm the diagnosis.

Heart failure

Breathlessness on exertion and lying flat, ankle swelling, and nocturnal awakening with breathlessness — a combination suggesting left ventricular failure. Hypertension is the leading cause in Lagos. ECG and chest X-ray are the first-line investigations.

Pneumonia and respiratory infection

Acute breathlessness with fever and productive cough — pneumonia. Examination of the chest distinguishes pneumonia from bronchitis.

Treatment

How We Treat It

Targeted at the identified cause:

  • Anaemia: Iron supplementation (or B12/folate as appropriate), malaria treatment if co-existing, investigate underlying blood loss
  • Asthma: Salbutamol inhaler + preventer (ICS) prescription, asthma action plan
  • Pneumonia: Appropriate antibiotics, hospitalisation if severe
  • Cardiac failure: Immediate hospital referral with ECG result and clinical summary
⚠️ Go to A&E immediately if: Breathlessness is severe at rest, lips or fingertips are turning blue, the person cannot speak in full sentences, or the person is a child who is breathing very fast with in-drawing of the chest between the ribs.

FAQs

Is breathlessness always a heart or lung problem?
No — anaemia, anxiety, deconditioning, obesity, and pregnancy can all cause breathlessness. A clinical assessment including FBC, chest examination, and ECG identifies the cause in the majority of cases.
My breathlessness has been getting worse for months. Is that concerning?
Progressive breathlessness over weeks to months warrants investigation regardless of age. In Lagos adults, the most common causes are worsening anaemia, developing asthma, or cardiac disease — all detectable and treatable at Mascot Healthcare.
