Studies consistently show that approximately 35% of Nigerian adults have hypertension. Of these, roughly 60% are unaware of their diagnosis, only 40% of those diagnosed are on treatment, and only 30% of those on treatment have their blood pressure controlled. This means that roughly 7 in 8 hypertensive Nigerians have uncontrolled blood pressure — silently damaging their organs.
The consequences are Nigeria's leading causes of hospital mortality: stroke (the #1 cause of adult neurological disability in Nigeria), heart failure, chronic kidney disease, and hypertensive emergencies.
Primary (essential) hypertension — with no single identifiable cause — accounts for 90–95% of cases. Risk factors prevalent in Lagos: high dietary salt (processed foods, stock cubes), obesity and abdominal fat, chronic stress from Lagos living conditions, physical inactivity, heavy alcohol use, and family history. Secondary hypertension (from kidney disease, aldosteronism, sleep apnoea) accounts for 5–10% — detectable with targeted testing.
First-line antihypertensives for Nigerian patients (based on current ISH Africa guidelines): ACE inhibitors (lisinopril, enalapril) or ARBs (losartan) — preferred in diabetics and for kidney protection; calcium channel blockers (amlodipine) — particularly effective in Black African patients; thiazide diuretics (hydrochlorothiazide) — low cost, effective. Most patients achieve target BP with combination therapy.
