Rheumatoid arthritis (RA) is an autoimmune disease in which the immune system attacks the joints, causing inflammation, damage, and deformity. Treatment Options

Early Treatment Is Critical
RA causes irreversible joint destruction if untreated. Starting disease-modifying treatment within 3 months of diagnosis significantly changes the long-term outcome โ€” this is a medical urgency, not an emergency.

Methotrexate โ€” First-Line DMARD
Weekly oral methotrexate is the cornerstone of RA treatment. It reduces inflammation, slows joint damage, and induces remission in many patients. Must be taken with folic acid. Regular blood monitoring required.

Other DMARDs
Hydroxychloroquine (Plaquenil), sulfasalazine, and leflunomide are used alone or in combination for moderate disease.

Frequently Asked Questions

Is rheumatoid arthritis the same as osteoarthritis?
No โ€” they are fundamentally different. Osteoarthritis is wear-and-tear damage. Rheumatoid arthritis is an autoimmune disease that causes inflammation throughout the body, not just in joints. The treatments are completely different.
Can RA be cured?
No โ€” but remission (minimal or no disease activity) is achievable in many patients with modern treatment. The goal is to control disease so completely that it has minimal impact on quality of life.
Is methotrexate chemotherapy?
Methotrexate is used in high doses as chemotherapy for cancer. In the very low doses used for RA, it is an anti-inflammatory medication โ€” not chemotherapy. The doses are 100x lower than cancer doses. It is safe and effective when properly monitored.
Can I exercise with RA?
Yes โ€” exercise is beneficial and recommended. Low-impact exercise (swimming, cycling, walking) maintains joint function and reduces fatigue. Exercise during flares should be gentler โ€” rest the affected joint but keep moving overall.
Does RA affect pregnancy?
RA often improves during pregnancy (immune modulation) but may flare post-partum. Many RA medications (methotrexate, leflunomide) are contraindicated in pregnancy. Plan pregnancy with your rheumatologist โ€” medication adjustment well before conception.
