Sickle Cell Trait (AS Genotype)

What it means to carry the sickle cell gene โ€” and what it doesn't mean.

Sickle cell trait (AS genotype) means you carry one copy of the sickle cell gene but do not have sickle cell disease. You cannot "develop" sickle cell disease. The most important implication of sickle cell trait is for family planning โ€” if both parents have trait, each child has a 25% chance of having sickle cell disease.

Signs and Symptoms

โœ“ Sickle cell trait does NOT cause sickle cell disease
โœ“ No symptoms under normal circumstances
โœ“ Under extreme conditions (severe dehydration, very high altitude, intense military or athletic training) โ€” rare complications can occur
โœ“ Increased risk of haematuria (blood in urine) from renal papillary necrosis โ€” usually benign
โœ“ Slightly increased risk of urinary tract infections during pregnancy
โœ“ NOT at risk of painful crises, acute chest syndrome, or stroke from trait alone

Risk Factors

When to Seek Help

RoutineKnowing your genotype before marriage or having children โ€” the most important action for anyone with sickle cell trait.
RoutinePre-pregnancy genetic counselling if your partner's genotype is AS, SS, or unknown.
SoonBlood in the urine โ€” usually benign in trait but should be assessed to exclude other causes.
SoonSevere or unusual muscle pain after extreme exercise โ€” rare but warrants assessment.

Tests & Diagnosis

๐Ÿงช Haemoglobin Electrophoresis / HPLC
Confirms AS genotype. Simple blood test available at Mascot Healthcare.
๐Ÿงช Partner Genotype Testing
Essential for family planning decisions. Both partners need to know their genotype.
๐Ÿงช Prenatal Testing (if AS + AS couple)
Chorionic villus sampling (CVS) or amniocentesis can identify fetal genotype โ€” allows informed decision-making if both parents are AS.

Treatment Options

1
No treatment required for trait itself
Sickle cell trait does not cause disease and requires no medication or special treatment.
2
Pre-marital and Pre-conception Genotype Counselling
The most important intervention. All couples in Nigeria should know both their genotypes before having children. This is a personal and family decision that prevents decades of suffering.
3
Stay Hydrated in Extreme Conditions
In rare circumstances (intense military training, extreme heat, high altitude) โ€” ensuring good hydration reduces the very small risk of complications.
4
Inform Medical Providers
Tell doctors and anaesthetists you have sickle cell trait before surgery, procedures, or starting new medications. It is usually of minimal clinical significance but should be documented.

Frequently Asked Questions

Can sickle cell trait become sickle cell disease?
No. Sickle cell trait (AS) cannot become sickle cell disease. You were born with your genotype and it does not change.
Am I healthy with sickle cell trait?
Yes โ€” the vast majority of people with sickle cell trait live entirely normal, healthy lives. The main implication is for your children, not your own health.
If my partner is also AS, what are our options?
You have several options: proceed with natural conception accepting the 25% risk with genetic testing of each pregnancy; use IVF with pre-implantation genetic diagnosis (PGD) to select unaffected embryos; or make other family planning decisions. Genetic counselling helps you understand all options without judgment.
Can I exercise and compete with sickle cell trait?
Yes. The vast majority of athletes with sickle cell trait compete at elite levels without complications. Adequate hydration, gradual training increases, and avoiding extreme conditions are prudent precautions.
Should children with sickle cell trait be told about it?
Yes โ€” ideally at an appropriate age. Understanding their genotype empowers them to make informed decisions about their own family planning when they are adults.
