Stroke Prevention in Nigeria
Reducing your stroke risk โ the evidence-based steps.
Stroke is largely preventable. Up to 90% of strokes globally are attributable to modifiable risk factors. In Nigeria, high blood pressure โ the leading cause of stroke โ affects the majority of adults over 50. This page explains what you can do, specifically in a Lagos context, to dramatically reduce your stroke risk.
Signs and Symptoms
โ Stroke prevention addresses risk factors rather than symptoms
โ Most stroke risk factors are silent โ high blood pressure, atrial fibrillation, high cholesterol have no symptoms
โ A previous TIA (transient ischaemic attack / "mini-stroke") is the most urgent reason to seek prevention care
โ Risk factors may include: hypertension, diabetes, atrial fibrillation, smoking, obesity, alcohol
When to Seek Help
NowAny TIA (transient ischaemic attack) โ symptoms that resolved completely. Go to hospital immediately โ the risk of full stroke in the next 48 hours is very high.
SoonBlood pressure consistently above 140/90mmHg โ this level significantly elevates stroke risk and needs medical management.
RoutineAnyone over 40 for a cardiovascular risk assessment โ to calculate 10-year stroke and heart attack risk and implement a prevention plan.
Tests & Diagnosis
๐งช Blood Pressure (Clinic + Home Monitoring)
24-hour ambulatory blood pressure monitoring provides the most accurate picture of true blood pressure burden โ including the critical nighttime reading. Nighttime hypertension is particularly strongly associated with stroke.
๐งช ECG (Electrocardiogram)
Identifies atrial fibrillation โ which requires anticoagulation to prevent embolic strokes. Annual ECG recommended from age 65; earlier if palpitations or pulse irregularity are noted.
๐งช Fasting Lipid Profile
LDL cholesterol target depends on individual risk. High-risk individuals (previous TIA, atrial fibrillation, diabetes) need LDL below 1.8 mmol/L.
๐งช Carotid Doppler Ultrasound
Identifies narrowing in the carotid arteries โ present in some people with TIA or stroke. Significant narrowing (>70%) can be surgically treated to prevent further events.
๐งช HbA1c
Uncontrolled diabetes is a major stroke risk factor. Target HbA1c below 7% in most people.
Frequently Asked Questions
What is the most powerful thing I can do to prevent stroke?
Control your blood pressure. If your blood pressure is consistently above 140/90mmHg and you are not on treatment, or above 130/80mmHg and you have diabetes or previous cardiovascular events โ starting treatment is the single highest-impact action you can take.
Does aspirin prevent stroke?
Aspirin is used after a stroke or TIA to prevent recurrence. For people who have never had a stroke, the bleeding risk of daily aspirin outweighs the benefit in most people. Do not self-prescribe aspirin for stroke prevention without medical advice.
Can I stop my blood pressure medication if I feel well?
No. Blood pressure medication controls, not cures, hypertension. Your blood pressure will rise again when you stop. Never stop blood pressure medication without discussing with your doctor first.
Is stroke hereditary?
Family history modestly increases stroke risk โ mainly through shared genes for hypertension, diabetes, and cholesterol. Having a first-degree relative with stroke should prompt earlier and more proactive risk factor management.
How do I know if I have had a TIA?
A TIA causes sudden neurological symptoms โ weakness, speech difficulty, vision loss, confusion โ that resolve completely within minutes to hours. Many TIAs are missed because they resolve before reaching hospital. If you have ever had such an episode, discuss it with a doctor โ even if you feel completely normal now.
