Transvaginal Ultrasound Scan (TVS)

A transvaginal scan (TVS) uses a slender probe gently inserted into the vagina to produce high-resolution images of the uterus and ovaries from close range. It is the most accurate ultrasound method for assessing fibroids, PCOS, ovarian cysts, the endometrium, and early pregnancy.

✅ Preparation Steps

Empty your bladder completely before the scan
Unlike an abdominal pelvic scan, the TVS requires an EMPTY bladder. A full bladder pushes the uterus away from the probe, reducing image quality. Use the bathroom immediately before entering the scan room.

No special fasting is required
You can eat and drink normally before a TVS. The only preparation is bladder emptying. You may take all regular medications as usual.

The scan can be done at any point in your menstrual cycle
A routine gynaecological TVS can be performed any time in the cycle. Exceptions: follicle tracking scans (for fertility) are timed to specific days; some endometrial assessments are best done in the early follicular phase (days 3–5). Your doctor's referral will specify if cycle-day timing is needed.

Wear comfortable clothing with easy lower-body access
You will remove clothing below the waist for the scan. A loose skirt or dress, or a wrap, makes this more comfortable and private. A modesty drape is always provided.

Inform us if you are pregnant or may be pregnant
TVS is safe in pregnancy and is actually the preferred method for early pregnancy scans (6–12 weeks). However, knowing you may be pregnant helps the sonographer interpret findings appropriately and communicate with sensitivity.

Preparation in room
You will be asked to undress from the waist down and lie on the scan couch with your knees bent and hips slightly raised, similar to a gynaecological examination position. A modesty drape covers your lower body at all times.
Probe insertion
The transvaginal probe is slim — narrower than a standard speculum used during a Pap smear. A clean disposable cover (probe cover) is placed on it, with a small amount of gel. The sonographer will insert it gently. Most women feel mild pressure — not pain. If you feel discomfort, let the sonographer know immediately.
Scanning (10–20 min)
The probe is gently moved to visualise the uterus (size, shape, endometrial thickness, cavity, fibroids), both ovaries (follicle count, cyst presence, PCOS appearance), and any free fluid in the pelvis. You may see the screen and the sonographer will explain key findings.
After the scan
The probe is removed and you can use the bathroom, dress, and collect your report. There is no recovery time. You may have a small amount of gel discharge — this is normal and resolves quickly.

Does a transvaginal scan hurt?
For most women, a TVS causes mild pressure — not pain. The probe is significantly narrower than a standard gynaecological speculum. Women who have never had penetrative sex or who have vaginal atrophy (post-menopausal) may find it more uncomfortable; in these cases, the sonographer can proceed very slowly or consider an alternative approach. Please communicate any discomfort throughout.
Can a TVS be done during my period?
Yes — a TVS can technically be performed during menstruation. The blood does not impair imaging. However, most women prefer to schedule it when they are not menstruating for comfort and cleanliness. If your clinical situation is urgent, do not delay because of your period.
Why is a TVS preferred over an abdominal scan for gynaecological problems?
The transvaginal probe sits millimetres from the uterus and ovaries, producing dramatically clearer images than the abdominal probe which scans through the abdominal wall and bladder. TVS can detect fibroids as small as 3–5mm, count antral follicles accurately for PCOS/fertility assessment, measure endometrial thickness precisely, and identify an early gestational sac at 5 weeks gestation — several weeks before the abdominal approach.
Do I need a TVS even if I already had an abdominal pelvic scan?
Often yes — especially if the abdominal scan gives an incomplete picture of the ovaries or endometrium, which is common in women with a retroverted uterus, obesity, or heavy bowel gas. The two scans complement each other and are frequently done as a combined assessment in the same appointment.
