4th-generation HIV test
A blood test that looks for both the HIV p24 antigen and HIV antibodies, so it picks up HIV earlier than an antibody-only test. A negative result 6 weeks (45 days) after a possible exposure is conclusive.
What the test looks for
After HIV enters the body, a part of the virus called the p24 antigen appears in the blood within a few weeks. The body's antibodies against HIV come later.
An antibody-only test (3rd generation, which includes rapid antibody tests) can only find HIV once those antibodies are there. A 4th-generation test looks for both, so it can find HIV earlier and gives a conclusive answer sooner.
4th-generation vs antibody-only test
| 4th-generation test | Antibody-only test (3rd generation) | |
|---|---|---|
| What it detects | p24 antigen and HIV antibodies | HIV antibodies only |
| Earliest it can detect HIV | About 14 to 18 days after exposure (lab blood test) | About 21 to 23 days after exposure |
| Finds 95% to 99% of infections by | 4 to 6 weeks (28 to 45 days) | 8 to 12 weeks |
| A negative result is conclusive at | 6 weeks (45 days) | 12 weeks (90 days) |
When to test
- 6 weeks (45 days) or more since the possible exposure: one 4th-generation test gives a conclusive answer.
- Less than 6 weeks: you can test now, and most infections show up from about 4 weeks. A negative result isn't conclusive yet, so test again once 6 weeks have passed.
- You took PEP or are taking PrEP: the timing is different. Ask our doctor when to test.
If you may have been exposed to HIV in the last 72 hours, come in today: emergency HIV prevention (PEP) works best within 72 hours.
What your result means
- Negative, 6 weeks or more after the exposure: you don't have HIV from that exposure.
- Negative, less than 6 weeks after the exposure: not conclusive yet. Test again at 6 weeks.
- Reactive: this is not a diagnosis on its own. It is always checked with a different test first (see HIV confirmatory testing), and our doctor will talk you through the result and the next steps.
Before your test
- No fasting needed.
- Know the date of the possible exposure, so we can tell you whether the result is conclusive.
- Tell us if you have taken PEP or are taking PrEP.
- Your result is confidential and is given only to you.
