Bleeding After Menopause?

Any vaginal bleeding that occurs 12 months or more after your last period is called postmenopausal bleeding — and it must always be investigated. In the majority of cases, the cause is benign. Atrophic vaginitis

After menopause, declining oestrogen causes the vaginal walls to thin, dry, and become fragile — leading to light bleeding, especially after sex. The most common cause of postmenopausal bleeding. Treated with topical oestrogen.

Most common cause — highly treatable

Endometrial polyps

Benign growths on the uterine lining. Cause irregular bleeding at any age but become more significant after menopause. Identified on transvaginal or pelvic scan.

Endometrial hyperplasia

Thickening of the uterine lining — often caused by unopposed oestrogen (from HRT, obesity, or PCOS). Can progress to cancer if untreated. Treated with progestogen or, for higher-risk types, surgery.

Endometrial cancer

Bleeding after menopause is the classic presenting symptom of uterine/endometrial cancer. It is the 4th most common cancer in Nigerian women and is highly curable when found early. This is why urgent assessment is essential.

Cervical causes

Cervical polyps or cervical cancer can also cause postmenopausal bleeding. A clinical examination of the cervix is part of the assessment.

HRT-related bleeding

Bleeding in the first 3–6 months of starting HRT can be normal as the body adjusts. Unexpected bleeding after this window warrants assessment.

I had some light spotting after menopause. Does it mean cancer?
The most common cause of postmenopausal bleeding is atrophic vaginitis — a benign, treatable condition. However, endometrial cancer must be excluded. The only way to do this is assessment — clinical examination + pelvic scan + endometrial thickness measurement. Most women assessed for postmenopausal bleeding do not have cancer. But the ones who do need to be found early.
