PCOS (Polycystic Ovary Syndrome)

The most common cause of irregular or absent periods after a negative pregnancy test. Often associated with weight change, acne, or facial hair. Requires a pelvic scan and hormonal blood tests.

Stress or Significant Weight Loss

Extreme psychological stress or rapid weight loss can suppress ovulation — causing a temporary missed period. Usually resolves when the trigger resolves.

Hyperprolactinaemia

Elevated prolactin (from a pituitary adenoma or medication) suppresses ovulation. Causes missed periods + sometimes milky nipple discharge. Requires a prolactin blood test.

Thyroid Disease

Both hypothyroidism and hyperthyroidism disrupt the menstrual cycle — weight gain or loss, fatigue, and missed periods. A TSH test identifies this.

Ectopic PregnancyURGENT

Positive pregnancy test + missed period + one-sided pain = ectopic pregnancy until proven otherwise by scan. This is a same-day scan emergency.
