๐ How to Prepare for a
Breast Ultrasound Scan
A breast ultrasound uses high-frequency sound waves to examine the internal structure of breast tissue. It is the first-line imaging test for breast lumps, pain, and nipple discharge in women under 40, and is used alongside mammography in older women. No radiation is involved, making it safe for all ages including pregnant women.

Preparation Steps
1
No fasting or dietary changes needed
Breast ultrasound requires no preparation related to food or drink. Eat and drink normally. Take all regular medications as usual.
2
Do not apply creams, lotions, powder, or deodorant to the breast or armpit area on the day
Any substance on the skin interferes with ultrasound gel and probe contact, creating air bubbles that reduce image quality. On the morning of your appointment, shower normally but do not apply deodorant, talc, body lotion, or any cream to the chest, breasts, or underarm area.
3
Wear a two-piece outfit for easy access
You will need to remove your top and bra for the scan. A two-piece outfit (separate top and bottom) is far more practical than a dress or jumpsuit. A front-opening top or bra is helpful but not essential.
4
The best time in your menstrual cycle is days 7โ14
Breast tissue is least dense and tender during the mid-follicular phase (days 7โ14 of the cycle, counting from the first day of your period). This gives the clearest images. Breast scanning during the premenstrual phase (days 21โ28) may be uncomfortable because breast tissue is more swollen and tender. If you have a symptomatic lump, do not delay for cycle timing, attend whenever the concern arises.
5
Note any symptoms before arriving
Be ready to tell the sonographer: the exact location and size of any lump you have noticed, how long it has been present, whether it changes with the menstrual cycle, and any associated nipple discharge (colour, which breast, which nipple position). This information directs the scan and ensures nothing is missed.
๐ What to Bring
- ๐ Doctor's referral or request form
- ๐ Previous breast scan reports or mammogram results
- ๐ Date of last menstrual period
- ๐ Family history of breast cancer if relevant (mention to sonographer)
- ๐ Any previous breast biopsy reports
๐ฌ What to Expect
Positioning
You will remove your top and bra and lie on the scan couch. For each breast, you will be asked to raise your arm above your head to flatten the breast against the chest wall and maximise the imaging area. The scan is done in both supine and oblique positions.
Gel and scanning (15โ25 min)
Warm ultrasound gel is applied over each breast. The probe is pressed firmly but comfortably across the entire breast in a systematic pattern, quadrant by quadrant, including the axillary tail (the breast tissue extending into the armpit). Each area is carefully examined and any lesion is measured in three dimensions.
BIRADS classification
The sonographer will classify any findings using the BIRADS (Breast Imaging Reporting and Data System) score: BIRADS 1โ2 = normal or benign; BIRADS 3 = probably benign (6-month follow-up); BIRADS 4 = suspicious (biopsy recommended); BIRADS 5 = highly suspicious for malignancy.
Report and next steps
A written report is prepared at the end of the scan. If a lesion is found, the report will specify its size, location, characteristics, and BIRADS category. For BIRADS 4 or 5, your doctor will arrange an urgent biopsy referral.
๐ After Your Test
- Wipe off the gel with tissue or a towel. It is water-based and leaves no stain.
- Resume all normal activities immediately: there are no restrictions after a breast ultrasound.
- Take your report directly to your referring doctor, especially if any lesion was identified.
- A BIRADS 2 (benign) finding does not require biopsy, but does require annual review if you are over 40.
- A BIRADS 3 (probably benign) finding requires a follow-up scan in 6 months to confirm stability, attend this appointment without delay.
- If you are under 35 and feel a lump, do not dismiss it because of your age. Breast cancer occurs in younger women too, and early detection at any age significantly improves outcomes.
Frequently Asked Questions
Is a breast ultrasound better than a mammogram?
They are complementary, not competitive. Ultrasound is better for: younger women with dense breast tissue (mammograms are less informative in dense breasts), evaluating a specific lump or area of concern, distinguishing solid masses from cysts, and guiding needle biopsies. Mammography is better for: detecting tiny calcifications (microcalcifications), screening older women with fatty breasts, and population-level cancer detection. Women over 40 with a breast symptom ideally have both tests.
Can a breast ultrasound detect cancer?
Yes, breast ultrasound detects the vast majority of breast cancers that present as a palpable lump. Cancers appear as irregular, hypoechoic, poorly defined masses, quite different from smooth, round, well-defined benign cysts. However, no imaging test guarantees 100% accuracy; a BIRADS 4 or 5 finding always requires tissue biopsy (core biopsy or fine-needle aspiration) for definitive diagnosis.
I have breast pain but no lump, do I need an ultrasound?
Breast pain (mastalgia) without a palpable lump is usually cyclical (related to hormonal fluctuations) and not caused by cancer. However, if the pain is persistent, localised to one spot, non-cyclical, or associated with skin changes or nipple discharge, imaging is appropriate. A normal scan provides reassurance; a directed scan at the area of pain occasionally finds an occult lesion.
What is a breast cyst and should I be worried?
Breast cysts are very common: fluid-filled sacs within the breast tissue. Simple cysts (round, thin-walled, entirely fluid-filled) are completely benign and require no treatment unless uncomfortable. Complex cysts (internal debris, thick walls, or solid components) require closer evaluation and occasionally aspiration or biopsy. The majority of breast lumps in women aged 20โ40 are either simple cysts or fibroadenomas, both benign.
