Mechanical (non-specific) back pain

The most common: 90% of cases. Caused by poor posture, muscle strain, lifting injury, or prolonged sitting. Typically worsens with movement, eases with rest. Treated with NSAIDs, muscle relaxants, and physiotherapy.

Disc herniation (slipped disc)

Kidney stones or infection cause flank pain (sides + back) that can be mistaken for musculoskeletal back pain. Key difference: kidney pain is colicky and radiates to the groin, associated with urinary symptoms.

Pain lasting more than 3 months. Requires a biopsychosocial assessment, physical, psychological, and social factors all contribute. Physiotherapy, lifestyle modification, and sometimes low-dose tricyclic antidepressants are used.
