Irritable Bowel Syndrome (IBS) in Nigeria

Chronic gut symptoms that keep returning โ€” what causes them and how to manage

Irritable bowel syndrome (IBS) is a functional gut disorder โ€” meaning there is no structural damage, infection, or disease visible on tests, yet the bowel behaves abnormally. It causes chronic abdominal pain, bloating, and altered bowel habits. IBS is very common in Nigeria โ€” particularly among young women, and people with high stress levels โ€” yet it is often dismissed or misdiagnosed. Understanding IBS is the first step to managing it.

Signs and Symptoms

โœ“ Recurrent abdominal pain at least 1 day per week, related to defaecation or associated with change in stool frequency/form
โœ“ Bloating โ€” often worse after meals and in the evening
โœ“ Alternating constipation and diarrhoea, or predominance of one
โœ“ Urgency to open bowels
โœ“ Sensation of incomplete bowel emptying
โœ“ Mucus in stools
โœ“ Symptoms triggered or worsened by food or stress
โœ“ Relief of pain after passing stool

Risk Factors

When to Seek Help

Age over 50 with new bowel symptomsNew IBS-like symptoms after 50 must be investigated to exclude colorectal cancer โ€” do not assume IBS.
Rectal bleeding or blood in stoolIBS does not cause bleeding โ€” this requires colonoscopy to exclude inflammatory bowel disease or colorectal cancer.
Unintentional weight lossNot a feature of IBS โ€” investigate for organic disease.
Nocturnal symptoms waking from sleepTrue IBS does not usually cause symptoms at night; symptoms waking you from sleep suggest organic pathology.

Tests & Diagnosis

๐Ÿงช Clinical diagnosis (Rome IV criteria)
IBS is diagnosed clinically if typical symptoms are present for 6 months with no alarm features. Investigations are used to exclude, not confirm, IBS.
๐Ÿงช Full blood count + CRP
Exclude anaemia and systemic inflammation โ€” both absent in IBS.
๐Ÿงช Stool for culture, ova, and parasites
Exclude infectious cause โ€” giardia and entamoeba histolytica are common in Nigeria and cause IBS-like symptoms.
๐Ÿงช Coeliac antibodies (anti-tTG IgA)
Coeliac disease mimics IBS โ€” especially diarrhoea-predominant IBS.
๐Ÿงช Colonoscopy
In older patients or those with alarm features โ€” to exclude IBD and colorectal cancer.
๐Ÿงช Hydrogen breath test
For lactose or fructose intolerance โ€” available at specialist GI centres.

Treatment Options

1
Dietary changes โ€” low FODMAP diet
FODMAPs are fermentable carbohydrates that worsen IBS symptoms. Common culprits in Nigeria: onions, garlic, beans, wheat, milk. A dietitian-guided low FODMAP trial for 6โ€“8 weeks significantly reduces symptoms in 60โ€“70% of IBS patients.
2
Soluble fibre for constipation-dominant IBS
Ispaghula husk (Fybogel/Metamucil) โ€” start low and increase gradually. Insoluble fibre (wheat bran) can worsen symptoms.
3
Antispasmodics for pain
Hyoscine butylbromide (Buscopan) or mebeverine taken 20 minutes before meals reduces intestinal cramping.
4
Loperamide (Imodium) for diarrhoea
Reduces bowel motility in diarrhoea-predominant IBS. Take before potentially triggering situations.
5
Treat underlying anxiety or depression
Psychological factors drive IBS in many patients. CBT (cognitive behavioural therapy) and gut-directed hypnotherapy have strong evidence for IBS. Treating anxiety often substantially improves gut symptoms.
6
Probiotics
Some strains (Lactobacillus, Bifidobacterium) modestly reduce IBS symptoms. Not all probiotics are equivalent. Try a single strain for 4 weeks and assess.
7
Peppermint oil capsules
Enteric-coated peppermint oil (Colpermin) reduces spasm and pain โ€” evidence supports short-term use.

Frequently Asked Questions

Is IBS the same as an ulcer?
No โ€” IBS is a functional disorder; there is no visible damage to the gut. An ulcer is a physical sore in the stomach or duodenum caused by H. pylori or NSAIDs. They have different symptoms, tests, and treatments.
Can stress cause IBS?
Stress does not cause IBS, but it significantly worsens it. The gut-brain axis means that psychological stress directly alters gut motility and sensitivity. Many people find their IBS flares during periods of anxiety or life stress.
Are beans a problem for IBS?
Beans are high-FODMAP foods (galactooligosaccharides) and commonly trigger bloating and gas in IBS. Canned beans rinsed with water are lower FODMAP than dried cooked beans. Many IBS patients need to reduce but not eliminate beans.
Can IBS turn into cancer?
No โ€” IBS does not progress to colorectal cancer or inflammatory bowel disease. Having IBS does not put you at higher risk of any GI cancer. However, it is important to ensure that your diagnosis is correct and no organic disease has been missed.
Is IBS curable?
IBS is a chronic condition but manageable. Many people achieve long periods of remission with dietary and lifestyle changes, stress management, and medication when needed. Symptoms often improve with age. It is not dangerous, but quality of life impact can be significant โ€” management is very much worth pursuing.
