Progesterone is produced by the corpus luteum after ovulation. Its level on day 21 (mid-luteal phase) is the best biochemical indicator of whether ovulation occurred — and whether the luteal phase is adequate to support early implantation.
In a standard 28-day Lagos menstrual cycle, ovulation occurs around day 14. Progesterone is sampled 7 days later (day 21) — at the mid-luteal peak. A result >30 nmol/L confirms ovulation. A low result (<16 nmol/L) indicates either anovulation or an incorrect sampling day.
Luteal phase defect (LPD) occurs when the corpus luteum produces insufficient progesterone to maintain the endometrial lining — causing implantation failure or early pregnancy loss. Lagos women with recurrent early miscarriage should have their luteal phase progesterone assessed before assuming structural or genetic causes.
Progesterone levels in early pregnancy should remain above 25–30 nmol/L (ACOG) throughout the first trimester to maintain the decidua. Falling progesterone in early Lagos pregnancy prompts intervention with vaginal progesterone pessaries — reducing miscarriage risk in women with a history of early pregnancy loss.

Supplemental progesterone significantly elevates blood levels and must be interpreted differently from endogenous production.
