Malaria — the first consideration in any febrile child in Lagos, particularly over 6 months when maternal immunity wanes. A malaria RDT takes 20 minutes and determines whether antimalarial treatment is needed.
Viral URTI — the most common overall cause of fever across all ages. Self-limiting, managed with paracetamol suspension (15mg/kg every 6 hours) and adequate fluids. Antibiotics are not needed.
Otitis media (ear infection) — very common in children, causes high fever and ear pulling. Diagnosed by otoscopy; treated with antibiotics in young children.
Urinary tract infection — particularly in girls under 5; UTI often causes fever without obvious urinary symptoms. A urine dip diagnoses it.
