Haemorrhoids โ€” called 'pile' in Nigeria โ€” are swollen veins in the lower rectum and anus. They are extremely common: half of all adults will have them at some point. Most cases are not dangerous and respond to diet and simple treatment. However, in Nigeria, the term 'pile' is often applied to every type of rectal symptom โ€” including rectal bleeding that may have a more serious cause. Correct diagnosis matters.

Treatment Options

High fibre diet and fluids
25โ€“35g fibre daily + 2 litres of water softens stools and eliminates the need to strain. This is the single most effective long-term treatment and prevents recurrence.

Sitz baths
Sit in a few inches of warm water for 15โ€“20 minutes, 2โ€“3 times daily and after each bowel movement. Relieves pain and spasm.

Topical preparations
Anusol, Germoloids, or witch hazel creams reduce itching and inflammation. Available in Nigerian pharmacies. Use for no more than 7โ€“10 days.

Rubber band ligation (RBL)
A rubber band is placed around the base of an internal haemorrhoid, cutting off its blood supply. It falls off within days. Effective for Grade Iโ€“III haemorrhoids. Done in outpatient clinic without anaesthesia.

Haemorrhoidectomy (surgery)
For severe Grade IIIโ€“IV haemorrhoids not responding to conservative treatment. The gold standard surgical procedure. Painful recovery (1โ€“2 weeks) but permanent cure.

LASER or stapled haemorrhoidopexy
Newer techniques available at some private hospitals in Lagos โ€” less painful recovery than conventional haemorrhoidectomy.

Frequently Asked Questions

What is the difference between internal and external haemorrhoids?
Internal haemorrhoids form inside the rectum โ€” usually painless but bleed. External haemorrhoids form under the skin around the anus โ€” can be painful, especially if a blood clot forms (thrombosed haemorrhoid). Many people have both.
Is pile the same as cancer?
No โ€” haemorrhoids are not cancer and do not turn into cancer. However, rectal bleeding attributed to 'pile' is sometimes actually from a rectal polyp or colorectal cancer. Any rectal bleeding should be assessed by a doctor, not assumed to be pile.
Are traditional pile treatments effective?
Some people use agbo (herbal preparations), castor oil, or other traditional remedies. There is no evidence these treat haemorrhoids effectively, and some may cause anal irritation. A high-fibre diet is far more evidence-based.
Can haemorrhoids go away on their own?
Small internal haemorrhoids often resolve with dietary changes and sitz baths. Larger or prolapsed haemorrhoids and thrombosed external haemorrhoids require treatment to resolve.
Will I need surgery?
Most haemorrhoids (Grade I and II) respond to dietary changes and rubber band ligation without surgery. Surgery (haemorrhoidectomy) is reserved for Grade IIIโ€“IV haemorrhoids that have failed other treatments โ€” a minority of cases.
