Swelling in the neck โ what it means and when to worry
A goitre is any enlargement of the thyroid gland. It is visible as a swelling at the front of the neck that moves up with swallowing. In Nigeria, goitre is common โ driven by iodine deficiency in inland areas, autoimmune thyroid disease, and thyroid nodules. Most goitres are benign, but any new or growing thyroid swelling must be assessed โ thyroid cancer, though rare, must be excluded.

Biopsy of a thyroid nodule using a thin needle โ the definitive test for thyroid cancer. Recommended for nodules above 1 cm with suspicious ultrasound features.

Frequently Asked Questions
Can cassava cause goitre?
Yes โ cassava contains cyanogenic glycosides (linamarin) that are converted to thiocyanate in the body. Thiocyanate blocks thyroid iodine uptake. Where both iodine deficiency and high cassava consumption co-exist (as in some Nigerian communities), the risk of goitre is particularly high. Ensuring adequate iodine intake mitigates this risk.
Is every neck swelling a goitre?
No โ the key distinguishing feature of thyroid swelling is that it moves upward with swallowing. Other neck swellings (lymph nodes, lipoma, branchial cyst, sarotid body tumour) do not move with swallowing. An ultrasound confirms the anatomy.
Is thyroid cancer common in Nigeria?
Thyroid cancer is relatively uncommon in Nigeria compared to Western countries, but incidence is increasing. Papillary thyroid cancer is the most common type โ well-differentiated and usually curable. Follicular thyroid cancer is associated with iodine deficiency. All suspicious thyroid nodules must be biopsied.
Can I feel my own thyroid?
A normal thyroid gland is usually not visible or palpable. You can check for goitre by: standing in front of a mirror, tilting your head back slightly, swallowing (take a sip of water), and observing the front of the neck for a bulge below the Adam's apple that rises with swallowing. Any visible swelling should be assessed.
Will my goitre go away?
It depends on the cause. Iodine deficiency goitre reduces with iodine supplementation. Hashimoto's goitre may partially respond to levothyroxine. Graves' goitre improves with treatment. Multinodular goitre (the most common type in older adults) does not resolve without intervention โ it may remain stable, grow slowly, or require surgery.
