Genital Herpes in Nigeria
A manageable but stigmatised condition โ the facts without shame
Genital herpes โ caused by herpes simplex virus type 2 (HSV-2) and sometimes HSV-1 โ is one of the most common sexually transmitted infections globally, affecting an estimated 500 million people. In Nigeria, HSV-2 prevalence is estimated at 30โ50% in adults. Despite being extremely common, herpes carries significant stigma. The facts: most infected people don't know they have it, it is manageable with medication, and it does not define a person's health or character.
Signs and Symptoms
โ First episode (often most severe): painful blisters or sores on or around the genitals, buttocks, or thighs; fever and flu-like symptoms; painful urination; swollen lymph nodes in the groin
โ Recurrent episodes: usually milder and shorter; tingling or itching (prodrome) 1โ2 days before blisters appear
โ Many people have MILD or NO symptoms despite infection โ and can still transmit
โ Herpes in women can be confused with vaginal infections or 'rashes'
โ Neonatal herpes: serious complication if transmitted during delivery โ can be fatal
When to Seek Help
First outbreak with blisters or ulcersConfirm diagnosis and start antiviral treatment within 72 hours for best effect on severity.
Frequent recurrences (more than 6 per year)Daily suppressive antiviral therapy dramatically reduces frequency.
Herpes in pregnancySpecialist management โ neonatal herpes is preventable with appropriate antiviral therapy and careful delivery planning.
Sores not healing or spreadingSevere or atypical herpes may indicate immunosuppression โ check HIV status.
Tests & Diagnosis
๐งช PCR swab from active lesion
The most sensitive diagnostic test โ swab from the base of a blister. Available at private labs and STI clinics.
๐งช HSV IgG serology
Detects past infection โ type-specific antibodies confirm HSV-1 or HSV-2 seropositivity. Useful when no active lesion is present.
๐งช HIV test
Herpes and HIV frequently co-exist โ test all patients diagnosed with herpes for HIV.
Frequently Asked Questions
Does having herpes mean my life is over?
No โ herpes is a manageable chronic condition. Most people live normal, full lives with herpes. With suppressive therapy, many people in serodiscordant relationships (one with herpes, one without) never transmit the virus. The medical impact of herpes is manageable; the social stigma is the harder part to address.
Can herpes be cured?
There is no cure โ HSV establishes latent infection in nerve ganglia and cannot be eliminated from the body. However, antiviral medications suppress the virus effectively, reducing outbreaks to near-zero in many people. Research into vaccines and gene therapy is ongoing.
Is oral herpes (cold sores) the same as genital herpes?
Both are caused by herpes simplex virus โ HSV-1 typically causes oral herpes (cold sores), HSV-2 typically causes genital herpes. However, HSV-1 can cause genital herpes (through oral-genital contact), and HSV-2 can cause oral herpes. Genital HSV-1 tends to have fewer recurrences than genital HSV-2.
Will I always have outbreaks?
No โ recurrence frequency typically decreases over years. Some people have frequent outbreaks (6โ12 per year) initially; others have rare or no recurrences after the first episode. Triggers include: stress, illness, sun exposure, menstruation, and immunosuppression.
Can herpes affect my fertility?
Genital herpes does not cause infertility. The primary concern is neonatal herpes โ if a woman has an active outbreak at delivery, a caesarean section is recommended to protect the baby.
