Red, ring-shaped, itchy rash on the groin, inner thighs, and sometimes the scrotum. Caused by dermatophyte fungi β the same organism as athlete's foot. Spreads easily in the heat and humidity of Lagos.
Antifungal cream or tabletsRedness, swelling, discharge, and soreness of the glans penis and/or foreskin. Caused by fungal infection (candida), bacterial overgrowth, poor hygiene, or skin conditions. More common in uncircumcised men.
Antifungal or antibiotic treatmentIntense itching β especially at night β at the pubic hairline, scrotum, or inner thighs. Visible nits or burrow tracks. Both are easily treated with topical insecticide, but household and partner treatment is essential.
Topical treatment β both partnersAllergic or irritant reaction to soap, shower gel, latex condoms, spermicide, or tight synthetic underwear. Causes localised redness, itching, and sometimes blistering.
Remove irritant + topical corticosteroidPainful blisters or ulcers on the penis, scrotum, or surrounding skin β often preceded by tingling or burning. Recurrent episodes are common. Antiviral treatment shortens outbreaks and reduces recurrence.
Antiviral treatment (aciclovir)Inflammatory skin conditions causing flat-topped, itchy, purple papules on the glans or shaft. May also affect the mouth. Requires topical steroid treatment and sometimes dermatology referral.
Topical steroid + dermatology referralLagos's heat, humidity, and urban density create ideal conditions for fungal skin infections. Tinea cruris (jock itch) and candida balanitis are extraordinarily common β significantly more so than in temperate climates. The combination of sweating, friction from clothing, and shared bathing facilities in urban housing creates a constant predisposition to fungal overgrowth.
The problem is compounded by the widespread practice of purchasing antifungal or antibiotic cream from pharmacies without diagnosis. When a bacterial condition (balanitis from Staphylococcus or gram-negative organisms) is treated with antifungal cream, it fails β and vice versa. A brief clinic examination identifies the organism category and directs the correct treatment.
