The most common cause of haematuria in men in Lagos. A stone in the kidney or ureter causes blood in the urine — often with severe flank pain. Abdominal scan identifies the stone size and location.
UTI in men is less common than in women but more significant — it almost always indicates a structural or predisposing cause (BPH, stone, structural abnormality). Blood in urine with burning and frequency in a man warrants full urological assessment.
Both BPH and prostate cancer can cause blood in urine — typically in older men. The blood is from engorged prostatic vessels or tumour vasculature. PSA + prostate scan is essential.
Bladder tumours (transitional cell carcinoma) classically present with painless haematuria in men over 50 — particularly smokers. Bladder stones can also cause haematuria. Abdominal scan assesses the bladder.
Intense exercise — particularly long-distance running — can cause transient haematuria (march haematuria) from bladder trauma. Usually resolves within 48 hours. Persistent haematuria after exercise requires investigation.
Men taking blood thinners (warfarin, aspirin, heparin) may develop haematuria at therapeutic levels — particularly if there is an underlying lesion. An underlying cause should still be excluded.
