Kidney stones cause some of the most severe pain in medicine โ€” described as worse than childbirth. They occur when minerals crystallise in the kidney and pass into the ureter (the tube leading to the bladder). Nigeria's hot climate and low fluid intake create ideal conditions for stone formation. Most stones pass without surgery, but prevention is essential to avoid recurrence.

Treatment Options

Analgesia (pain relief)
NSAIDs (diclofenac, ibuprofen) are more effective than opioids for renal colic by reducing ureteric spasm. IV ketorolac in hospital for severe pain. Avoid NSAIDs in people with kidney disease.

Hydration
Drink 2โ€“3 litres of fluid daily โ€” more in Nigeria's heat and after exercise. This both treats current stones (flushes them) and prevents future ones.

Alpha-blockers (tamsulosin) for stone passage
Tamsulosin relaxes the ureter and significantly increases the chance of stone passage for stones 5โ€“10 mm. Available in Nigeria.

Watchful waiting for small stones (<5 mm)
80โ€“90% of small stones pass spontaneously within 4 weeks with adequate hydration and analgesia.

Ureteroscopy
A camera-guided instrument removes or breaks the stone under general anaesthesia. The commonest interventional procedure for ureteric stones in Nigeria.

Lithotripsy (ESWL)
Shock waves break up stones non-invasively โ€” available at some Nigerian urology centres.

Long-term prevention
Drink 2โ€“3 litres water daily. Reduce salt. Reduce animal protein. Maintain normal calcium intake (low calcium diet paradoxically increases oxalate stones). If recurrent: metabolic workup and specific prevention (thiazide diuretics for calcium stones; allopurinol for uric acid stones).

Is the pain from kidney stones really as bad as described?
Yes โ€” renal colic from kidney stones is consistently rated as some of the worst pain experienced in medicine. It occurs because ureteric spasm and distension of the collecting system are intensely painful. Most people who have experienced both describe it as worse than surgical postoperative pain.
Can I dissolve kidney stones with lemon juice?
Lemon juice (citric acid) can dissolve uric acid stones by alkalinising urine โ€” there is modest evidence for lemon water reducing recurrence of calcium oxalate and uric acid stones. It cannot dissolve a large stone acutely. Drink diluted lemon water (half a fresh lemon in 1 litre water) daily as part of prevention, not as sole treatment.
Will I always have kidney stones once I have had one?
50% of people who have one kidney stone will have another within 5 years without prevention measures. With proper hydration, dietary changes, and medication where needed, recurrence can be dramatically reduced.
Are kidney stones and kidney disease related?
Recurrent kidney stones can cause kidney damage (obstruction and scarring). People with recurrent stones should have kidney function monitored. Kidney disease (CKD) also increases the risk of certain types of kidney stones. They can co-exist and affect each other.
Do Nigerians have more kidney stones because of the heat?
Yes โ€” hot climates lead to increased sweating and concentrated urine, which is a major risk factor for stone crystallisation. The most important countermeasure is aggressive fluid intake โ€” 2.5โ€“3 litres daily, more when sweating heavily.
