Tuberculosis (TB) in Nigeria

TB is curable โ€” but stigma and delayed diagnosis cost lives.

Nigeria has one of the highest TB burdens globally โ€” ranked sixth worldwide by the WHO. TB is caused by Mycobacterium tuberculosis and spreads through the air. It is curable with a full 6-month treatment course. The greatest barriers to cure are late presentation (driven by stigma and misdiagnosis) and treatment non-completion.

When to Seek Help

Tests & Diagnosis

๐Ÿงช Sputum Smear (AFB)
Microscopy of sputum for acid-fast bacilli โ€” cheap, widely available, but misses up to 50% of TB cases. Three samples on different days improves sensitivity.
๐Ÿงช GeneXpert MTB/RIF
Rapid molecular test โ€” results within 2 hours, sensitive (85โ€“90%), specific, and detects rifampicin resistance simultaneously. Now widely available in Lagos through DOTS centres and reference labs.
๐Ÿงช Sputum Culture (Lowenstein-Jensen or MGIT)
Gold standard โ€” most sensitive but takes 2โ€“8 weeks for results. Also provides drug susceptibility testing.
๐Ÿงช Chest X-ray
Upper lobe infiltrates, cavitation, and lymphadenopathy are classic โ€” but TB can have atypical appearances, especially in HIV-positive patients.

Mandatory in all new TB diagnoses โ€” co-infection dramatically changes management.
๐Ÿงช Tuberculin Skin Test (TST) / IGRA
Tests for TB infection (latent TB) rather than active disease โ€” used for contact tracing and screening of healthcare workers.

Treatment Options

Standard 6-Month DOTS Regimen
2 months of RIPE (Rifampicin, Isoniazid, Pyrazinamide, Ethambutol) then 4 months of RI (Rifampicin, Isoniazid). Available FREE through the Nigerian National DOTS programme in public health facilities.

Treatment Completion Is Non-Negotiable
Stopping early โ€” even after symptoms resolve โ€” causes treatment failure and drug-resistant TB. Drug-resistant TB (MDR-TB) requires 18โ€“24 months of toxic second-line medications and has a much lower cure rate.

HIV and TB Co-treatment
Both conditions must be treated simultaneously. Antiretroviral therapy is started within the first 8 weeks of TB treatment. Drug interactions between rifampicin and ARVs require specialist management.

Directly Observed Therapy (DOTS)
A healthcare worker or trained supporter watches the patient take each dose. Dramatically improves treatment completion rates. The standard of care in Nigeria.

Drug-Resistant TB (MDR-TB)
Treated with second-line drugs (bedaquiline, linezolid, newer regimens) for 6โ€“24 months at specialised MDR-TB treatment centres.

Frequently Asked Questions

Is TB contagious?
Yes โ€” TB spreads through the air when an infectious person coughs, sneezes, or speaks. However, casual contact (touching, sharing utensils) does not spread TB. Close household contacts are at highest risk and should be tested.
Is TB treatment free in Nigeria?
Yes โ€” through the National DOTS programme, TB diagnosis and treatment are provided free of charge at public health facilities. Private clinics can diagnose and refer to DOTS centres for free treatment.
I have been on TB treatment for 2 months and feel much better โ€” can I stop?
Absolutely not. You must complete the full 6-month course even if you feel completely well. The bacteria are still present in reduced numbers. Stopping early causes relapse and develops resistant TB, which is much harder to treat.
Can TB be cured completely?
Yes โ€” drug-sensitive TB is curable in 95%+ of cases with a complete 6-month treatment course. Drug-resistant TB has lower cure rates but can still be treated.
What is latent TB?
Latent TB means TB bacteria are present in the body in a dormant state โ€” not causing disease, not contagious. Latent TB has a 5โ€“10% lifetime risk of becoming active TB. People with HIV, diabetes, or on immunosuppressants who have latent TB are offered preventive treatment.
