The signs that must not be missed — and why Nigerians delay treatment

Every minute after a heart attack begins, more heart muscle dies. The Nigerian average time from symptom onset to reaching hospital is 6–12 hours — compared to 1–2 hours in the UK. This delay is killing people. The three reasons: not recognising the symptoms, attributing them to spiritual causes, and not knowing where to go. This guide addresses all three.

Any suspected heart attack symptomsDo not wait to see if it improves. Do not drive yourself. Call for help immediately. Time is heart muscle.
Symptoms lasting more than 5 minutesAngina resolves within minutes with rest. Pain lasting beyond 5 minutes at rest is a heart attack until proven otherwise.
GTN (nitrate spray) not working after 2 dosesIf chest pain persists after 2 doses of sublingual GTN, assume heart attack and go to hospital.

Tests & Diagnosis

🧪 ECG within 10 minutes of arrival
The first and most important test. STEMI (ST-elevation) is an emergency requiring immediate reperfusion.
🧪 High-sensitivity troponin
Detects heart muscle damage. A rising level confirms heart attack.
🧪 Chest X-ray
Checks for pulmonary oedema.
🧪 Echocardiogram
Assesses wall motion — confirms area of damage and ejection fraction.
🧪 Coronary angiogram
Identifies the blocked artery for stenting (PCI) — ideally within 90 minutes of arrival for STEMI.

Treatment Options

IMMEDIATELY: Chew aspirin 300 mg
Chewing (not swallowing whole) gives faster absorption. This begins to dissolve the clot. Do not take if allergic to aspirin.

Call for help and go to hospital NOW
The closest hospital with a cardiac unit. In Lagos: Reddington Hospital, Lagos Island General Hospital, LASUTH, LUTH. Do not wait for morning. Do not drive yourself.

Primary PCI (coronary stenting)
Opening the blocked artery with a balloon and stent within 90 minutes is the gold-standard treatment. Available at Lagos, Abuja, Port Harcourt, and Kano cardiac centres.

Thrombolysis (clot-busting drugs)
If PCI is not available within 120 minutes — streptokinase or alteplase dissolves the clot chemically. Available at many teaching hospitals. Less effective than PCI but dramatically better than nothing.

Post-MI medications for life
Aspirin, statin, ACE inhibitor, and beta-blocker — the standard four drugs started before hospital discharge. Each one reduces the risk of a second heart attack.

Cardiac rehabilitation
6–12 weeks of supervised exercise, education, and psychological support after heart attack. Reduces second heart attack risk by 25%. Ask your cardiologist for referral.

Frequently Asked Questions

Why do Nigerians reach hospital so late after a heart attack?
Multiple studies confirm: symptoms attributed to spiritual attack, chest pain seen as 'gas', not knowing which hospital to go to, transport challenges, financial concern about hospital costs, and waiting for family consensus. All of these cost lives. Having a plan before the emergency occurs is the solution.
Is heart attack the same as cardiac arrest?
No — a heart attack is a blocked coronary artery causing heart muscle death. The heart is still beating. Cardiac arrest is when the heart stops beating entirely — the person loses consciousness and has no pulse. A heart attack can cause cardiac arrest. Cardiac arrest requires immediate CPR.
Can a woman have a heart attack without chest pain?
Yes — women more frequently present with atypical symptoms: jaw pain, nausea, fatigue, breathlessness, or shoulder pain without classic central chest pressure. This leads to delayed diagnosis in women. Any woman with these symptoms plus cardiac risk factors should be evaluated urgently.
What should I do if someone collapses at home after chest pain?
Check for response and normal breathing. If not breathing normally, begin CPR immediately (see CPR bystander guide). Call for help. Do not leave them alone. If they are breathing and responsive, put them in the recovery position and call an ambulance or rush to hospital.
How long does recovery take after a heart attack?
Most people are discharged within 3–5 days after uncomplicated PCI. Return to light activity within 1–2 weeks. Return to driving and light work within 4–6 weeks. Full recovery and return to exercise within 6–12 weeks with cardiac rehabilitation. Emotional recovery (depression is common after MI) takes longer — address with support.
