What to do when someone collapses โ€” hands-only CPR can save a life

When someone's heart stops (cardiac arrest), brain death begins within 4โ€“6 minutes without oxygen. The chance of survival decreases by 10% for every minute without CPR. In Nigeria, bystander CPR is rarely performed โ€” largely because most people have never been taught. This guide teaches the basic steps of hands-only CPR that anyone can perform to double or triple a cardiac arrest victim's chance of survival.

Signs and Symptoms

STEP 1: Check for danger
Is it safe to approach? Is there still an electrical source? Generator fumes? Move the person only if necessary.

Tap shoulders firmly and shout 'Are you okay?' If no response โ€” proceed immediately.

STEP 3: Call for help
Shout for bystanders. Send someone to call emergency services or arrange transport to hospital. Dial Lagos State Emergency: 767 or 112.

STEP 4: Check breathing
Look for chest rise for no more than 10 seconds. Gasping or no breathing = cardiac arrest. Proceed to CPR.

STEP 5: Start chest compressions (Hands-Only CPR)
Place the heel of one hand on the centre of the chest (lower half of the breastbone). Place your other hand on top, fingers interlocked. Straighten your elbows and compress straight down: depth of 5โ€“6 cm (about 2 inches). Rate: 100โ€“120 compressions per minute (the beat of 'Stayin' Alive' by Bee Gees or 'Don't Stop Me Now' by Queen). ALLOW FULL CHEST RECOIL between compressions.

STEP 6: If trained in rescue breaths (30:2)
After 30 compressions โ€” tilt head, lift chin, pinch nose, and give 2 rescue breaths over 1 second each. If not trained or unwilling, hands-only CPR is equally effective for adults. Continue: 30 compressions, 2 breaths.

STEP 7: Continue until help arrives
Do not stop to check for pulse. CPR is exhausting โ€” if multiple bystanders, swap every 2 minutes.

STEP 8: AED (Automated External Defibrillator)
If available (increasingly found in airports, large shopping malls, and some hospitals) โ€” turn it on and follow its voice instructions. The AED will shock only if needed; it cannot harm a person in cardiac arrest.

Can I hurt someone by doing CPR?
Rib fractures are possible from effective compressions โ€” but a person in cardiac arrest is already dying. The risk of rib fracture is far less important than the risk of death from no CPR. Do not withhold CPR for fear of causing injury. Effective CPR requires firm, deep compressions โ€” you will feel resistance and possibly hear sounds from the chest; continue.
What if I don't know how to give rescue breaths?
Hands-only CPR (compressions only, no rescue breaths) is as effective as CPR with breaths for adult cardiac arrest. The blood retains enough oxygen for the first few minutes of CPR. If you are untrained, uncomfortable, or the patient has a contagious infection, give compressions only.
How do I know if someone is in cardiac arrest or just fainted?
Fainting: the person recovers consciousness quickly when laid flat; is breathing normally; wakes up confused but responsive. Cardiac arrest: the person does not regain consciousness; is not breathing normally or is gasping; does not respond to voice or touch. Start CPR if in doubt โ€” you cannot make things worse.
What are the emergency numbers in Nigeria?
Lagos State Emergency: 767 or 112. National Emergency: 112 (active in some states). Also call: Federal Road Safety Corps (FRSC): 122. Ambulance services are limited โ€” in many areas, bystander transport to hospital while CPR continues is the only option.
Is CPR effective in Nigeria without a defibrillator?
Yes โ€” bystander CPR buys time, maintaining minimal circulation to the brain and vital organs until a defibrillator or advanced medical care arrives. High-quality bystander CPR doubles or triples survival rates even without a defibrillator. Start immediately.
