Condition Comparison

Malaria vs Typhoid

Same symptoms, different conditions โ€” here's exactly how to tell them apart.

Malaria

A parasitic infection (Plasmodium falciparum in Nigeria) transmitted by infected female Anopheles mosquitoes. One of the leading causes of illness in Lagos.

VS

Typhoid

A bacterial infection caused by Salmonella typhi, spread through contaminated food or water. Increasingly common in urban Nigeria due to poor water sanitation.

The Key Difference

๐Ÿ’ก How to tell them apart

Onset speed and fever pattern are the best clinical clues. Malaria hits fast โ€” a healthy person becomes severely unwell within hours to a day, with intense rigors (uncontrollable shaking) and a cyclical fever that spikes and breaks. Typhoid fever builds gradually over 3โ€“7 days, climbing higher each day in a "stepladder" pattern, with more prominent abdominal complaints. However, in practice these distinctions blur โ€” many patients present with both conditions simultaneously. Never treat empirically for malaria or typhoid without a test. Antibiotic resistance in Salmonella typhi is rising in Nigeria; guessing the wrong diagnosis wastes time and creates resistance.

Which Tests Diagnose Each?

๐Ÿงช Malaria Rapid Diagnostic Test (RDT)
A finger-prick blood test that detects malaria antigens in 15โ€“20 minutes. Highly accurate for falciparum malaria. Available at Mascot Healthcare same-day.
๐Ÿงช Malaria Thick and Thin Blood Film
Microscopy to visualise the parasite and quantify parasite load (parasitaemia %). The gold standard; useful for species identification and monitoring treatment response.
๐Ÿงช Widal Test (Typhoid Serology)
Detects antibodies against Salmonella typhi antigens. A titre of 1:160 or higher is considered significant in Nigeria, though false positives occur in endemic areas. Best interpreted in clinical context.
๐Ÿงช Blood Culture (Typhoid Gold Standard)
Grows Salmonella typhi from a blood sample โ€” 80โ€“90% sensitive in the first week of illness. Takes 3โ€“5 days. Used for definitive diagnosis and antibiotic sensitivity testing.
๐Ÿงช Full Blood Count (FBC)
In malaria: often low platelets (thrombocytopenia) and anaemia. In typhoid: characteristically low or normal white cell count (leucopenia) โ€” a useful clue.

Frequently Asked Questions

Can I have malaria and typhoid at the same time?
Yes โ€” co-infection is frequently seen in Nigeria. Both conditions cause fever and can occur together after a single high-risk exposure period (e.g., dirty water + mosquito exposure). This is why testing for both at the same visit is always recommended when fever is present.
Is the Widal test reliable for typhoid?
The Widal test is widely used in Nigeria but has significant limitations: it can give false positives after previous typhoid infection or vaccination, and false negatives early in illness. A high rising titre on two samples taken 7โ€“10 days apart is more meaningful than a single result. Blood culture remains the gold standard.
Can I treat myself at home for malaria?
Self-treatment without a confirmed test is not recommended, even in a malaria-endemic region. Not every fever is malaria; taking ACTs (artemisinin-based combination therapy) unnecessarily contributes to drug resistance. A rapid test at the clinic takes 20 minutes and removes all guesswork.
How soon should I see a doctor for fever?
Any fever above 38.5ยฐC lasting more than 24 hours, especially with rigors or confusion, should be evaluated the same day. Malaria can progress to severe (cerebral) malaria rapidly. Children and pregnant women should seek care immediately with any fever.
