Rheumatoid Arthritis in Nigeria

Understanding and managing inflammatory joint disease in Nigeria

Rheumatoid arthritis (RA) is a chronic autoimmune disease that causes painful inflammation of joints โ€” primarily the small joints of the hands and feet โ€” leading to progressive joint destruction, deformity, and disability if untreated. It affects approximately 0.5โ€“1% of the global population and is 2โ€“3 times more common in women. In Nigeria, RA is frequently diagnosed late โ€” often after years of seeing multiple providers, being misdiagnosed as joint pain, typhoid, or 'body aches', and receiving ineffective traditional or symptomatic treatments. Early treatment with disease-modifying drugs (DMARDs) prevents joint destruction and preserves function.

Signs and Symptoms

โœ“ Morning stiffness lasting more than 1 hour (cardinal symptom โ€” distinguishes RA from osteoarthritis)
โœ“ Symmetrical joint swelling: same joints on both sides (e.g., both wrists, both finger joints)
โœ“ Tender, warm, swollen small joints: metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints of hands, wrists, feet
โœ“ Grip weakness and difficulty with fine tasks (buttoning clothes, opening jars)
โœ“ Fatigue โ€” often severe and underappreciated
โœ“ Low-grade fever and weight loss during active disease
โœ“ Rheumatoid nodules: firm lumps under the skin near elbows (seropositive RA)
โœ“ Extra-articular features: dry eyes (Sjogren's), anaemia, lung nodules, vasculitis (less common)

Risk Factors

When to Seek Help

Symmetrical joint swelling lasting more than 6 weeksThis is the key diagnostic window โ€” early rheumatology referral and DMARD treatment within 3 months of symptom onset significantly improves long-term outcomes
Morning stiffness > 1 hourProlonged morning stiffness is the hallmark of inflammatory arthritis โ€” distinguish from the brief stiffness of osteoarthritis
Wrist swelling or grip weakness developing over weeksThe wrist is the most commonly affected joint in early RA โ€” don't attribute it to strain
Joint symptoms + persistent fatigue + anaemiaThe systemic nature of RA โ€” get rheumatoid factor and anti-CCP tested
Known RA with new eye redness or drynessScleritis or Sjogren's syndrome โ€” eye complications of RA need ophthalmology review

Tests & Diagnosis

๐Ÿงช Rheumatoid factor (RF)
Positive in 70โ€“80% of RA cases โ€” but also positive in other conditions. Not specific alone
๐Ÿงช Anti-CCP antibody (ACPA)
More specific than RF (95% specificity) โ€” highly predictive of erosive, progressive disease. A key diagnostic test
๐Ÿงช ESR and CRP
Elevated in active inflammation โ€” used to monitor disease activity and treatment response
๐Ÿงช Full blood count
Normocytic anaemia of chronic inflammation; platelets often elevated in active RA
๐Ÿงช X-rays of hands and feet
At diagnosis โ€” periarticular osteopoenia and joint space narrowing in early RA. Erosions in established disease
๐Ÿงช Ultrasound of joints
More sensitive than X-ray for early synovitis and erosions โ€” available at some Nigerian rheumatology centres
๐Ÿงช Liver and kidney function
Before starting DMARDs โ€” methotrexate is hepatotoxic; NSAIDs affect kidneys

Treatment Options

1
Methotrexate โ€” first-line DMARD
7.5โ€“25mg once weekly (oral or SC injection). Most effective and widely available DMARD. Requires folic acid 5mg weekly (not same day as methotrexate). Monitor LFTs and FBC every 4โ€“6 weeks initially. Avoid in liver disease, heavy alcohol use, or pregnancy
2
Hydroxychloroquine
Used in combination or for milder RA. Well-tolerated, requires annual eye check
3
Sulfasalazine
Often used in combination with methotrexate โ€” effective and available in Nigeria
4
Prednisolone (bridging therapy)
Short courses during flares while waiting for DMARDs to take effect. Avoid long-term use due to serious side effects
5
Biologic agents (TNF inhibitors)
Etanercept, adalimumab, infliximab โ€” highly effective for refractory RA. Expensive and limited availability in Nigeria. Requires TB screening before starting (reactivates latent TB)
6
Physiotherapy
Essential for maintaining range of motion and muscle strength โ€” often neglected but critical for function
7
Pain management
NSAIDs (ibuprofen, diclofenac) for pain and stiffness. COX-2 inhibitors if GI intolerance. Avoid long-term opioids

Frequently Asked Questions

Is rheumatoid arthritis the same as osteoarthritis?
No. Osteoarthritis (OA) is wear-and-tear cartilage degeneration โ€” common in elderly, affects weight-bearing joints, stiffness lasts < 30 minutes. RA is autoimmune โ€” affects small joints symmetrically, stiffness lasts > 1 hour, occurs in younger people, and causes systemic symptoms. The treatment approaches are completely different.
Can I still work with rheumatoid arthritis?
With early diagnosis and effective DMARD treatment, most people with RA maintain employment. Occupational therapy adaptations, ergonomic equipment, and flexible working arrangements can help. Delayed or inadequate treatment leads to the joint destruction and disability that forces people out of work.
Is methotrexate a chemotherapy drug?
Yes โ€” methotrexate is used in cancer chemotherapy at much higher doses. At the low weekly doses used in RA (typically 10โ€“20mg/week vs 1000mg+ in cancer), it acts as an anti-inflammatory immunosuppressant. The side effect profile at these doses is manageable with monitoring.
Where can I get specialist treatment for RA in Nigeria?
Rheumatology clinics are available at UCH Ibadan, LUTH Lagos, UNTH Enugu, Aminu Kano Teaching Hospital, National Hospital Abuja, and some private practices. Most DMARDs are available but biologics remain expensive and largely out of reach without donor support.
